Recognizing the possibility of physical injury associated with Miss Quitadamo's CampQuest Walk,, I hereby release, discharge and/or otherwise indemnify Foley Stadium, Worcester Public Schools, its affiliated organizations, including, but not by way of limitation, the facilities used for the program, against any claim by me or on behalf of my child as a result of my child's participation in the walk.

 

My child has received a physical examination by a registered physician and has been found physically capable of participating in the walk.  I hereby give my consent to have an athletic trainer, emergency medical technician and/or a doctor of medicine or dentistry provide my child with medical assistance and/or treatment and agree to be financially responsible for the reasonable cost of such assistance or treatment.

 

I authorize my photograph, picture or likeness, and voice to appear in any documentary, promotion (including advertising), television, video, or radio coverage of the league or tournament, without compensation.
